TN738 : Risk Management Dimensional Stone Mines by Using Fuzzy Fault Tree Analysis Approach

Abstarct: Mining is one of the most dangerous activities around the world that is always associated with various incidents, injuries, loss of life and damage. There are a number of safety, health and environmental hazards in dimensional stone mines that negate and lack planning to control them can have irreparable consequences. Therefore, risk assessment is important in these mines. The present thesis aims is preventing the occurrence or recurrence of costly incidents in the form of providing a model for risk management with identifying risks, assessing, rating and controlling risks at acceptable levels, so that it can have a significant impact on the identification of the root causes of events and the transfer of lessons from incidents to other mineral units. After conducting studies and collecting information about the causes of the risks, Judgment through the questionnaire was carried out by the relevant experts and then all the causes of the incident are identified and arranged in the form of Top, Intermediate and Basic Events in a structure up to the bottom of the tree, in four main categories of hazards related to machinery, electrical equipment, manpower and wire cutting machine. Considering the tree model, we used the fuzzy theory to weigh the root causes in the end, the probability of occurrence for each Top Event and Basic Event can be achieved. Then, the fuzzy Delphi hierarchy analysis method was used to calculate the severity of the outcome of each of the Top Events and a risk matrix for eventual events in dimensional stone mines. The results show that the most critical risks are hazards related to manpower, Subsequently, the risks associated with the cutting machine, machine risks and electrical equipment risks were lower. Finally, due to the control methods, appropriate responses for risk control are presented.
