Abstarct: The performance evaluation is one of the most important today's organizations actions, especially in case of employers, for delivering the works to decent contractors. For processing of the employers successful evaluation performance, besides employing the best and most efficient evaluation tools, it needs to design an appropriate and special assessment indicators that leads the indicators, strategies, missions, vision and values of the organization all to be in one line. The design evaluation process of the most organizations are mainly relying on iterative process of trial and error which has very low reliability in identifying the performance targets, the requirements to achieve goals, and choosing the best solution to achieve goals. In this research the researcher attempts to consider the petrochemical companies contractors' performance evaluation process importance for the first time to present a comprehensive and complete evaluation and judgment by using the Fuzzy analytic network process (FANP) and 360 degree feedback methods by which it can be helpful on saving money and reducing the time of assigned projects of the contractors. To reach this objective, several interviews have been done with the 11 managers and reporters of Jam petrochemical company about identifying indexes and sub-indexes, so finally for identifying the importance level of these indexes and sub-indexes, some questionnaire forms had been distributed among them, filled by them, therefore all the collected information had been used for the model implementation. The designed questionnaires had high reliability and by having the speed and simplicity in design have much higher Cronbach's alpha coefficient than other questionnaires. For avoiding the high human error possibility in determining and judgment on the contractors evaluation item in this research, the 360 feedback method is being used on this regard where accordingly a survey has been done with all the persons related to the contractors. In this research we pay attention to Jam petrochemical company's contractors' evaluation designing method, by which finally one of the contractors as an case study, will be measured and evaluated
