Abstarct: Accurately identifying and assessing the risk of implementing new technologies for organizations and responding appropriately to them, is critical throughout the technology life cycle. Blockchain technology has been introduced as one of the emerging technologies in the field of information technology in recent years. The diverse benefits of blockchain technology, such as improved process efficiency and effectiveness, have led system developers to embrace this technology. However, implementing new technology for the organization has always been risky. In this study, to identify the risks of blockchain implementation, the risks have been identified and categorized baxsed on the subject literature. Then, by performing a fuzzy Delphi process in two stages, the final summary of risks is performed baxsed on the experts’ opinions. In the following, causal relationships between risks are identified and analyzed using the fuzzy mapping method. Finally, the most important risks are ranked baxsed on the degree of prominence and the relationships between them. The industry of integrated organization resource management system development has been studied as a case study. The key results of the study indicate that among the risks, the immaturity of the technology was the most effective, the investment cost was the most impressive risk and privacy had the highest value of the risk centrality index; Also, the high investment cost has the highest priority among other risks, as well as privacy and contract law issues are ranked second, and third, respectively.
