Abstarct: The increasing development of mining and occurrence of some incidents and different issues caused the augment of mining risk for many reasons. In this thesis, the Faryab chromite mines safety risk has been investigated. The safety risk assessment for 7 mines from Faryab mining complex using hierarchal analysis method at fuzzy sets of 1 and 2 has been carried out. In assessment of these risks using hierarchal analysis method at fuzzy sets of 1, making decision for selecting final risks has some faults and thus for the first time, it was used hierarchal analysis method at fuzzy sets of 2 in this thesis. The work process has followed in this thesis in a such way that in preliminary inspections of mine, the existing documents has been reviewed and the interview from 194 people in open and underground mines has been done and 203 risks has been detected. Using Pareto analysis of 35 percent of the views of people in early questionnaire, 72 risks has been regarded as frequent risks. After categorizing of 72 detected risks in open and underground mines, the designing and distribution of questionnaire among 20 members of technical personnel, the rating of risks has been carried out. By assessment of technical personnel questionnaire using hierarchal analysis method at fuzzy sets of 1 and 2 and comparing their results, 26 risks in open and underground mines regarded as the most important risks. Ultimately, we specified 8 risks in open and underground mines by calculating final weight average of each risk in these two methods. In open mines, two risks include remaining naryad material in rock and getting on people in unsafety vehicles have the final weight 0.470 and 0.360, respectively and in underground mines, there are 3 risks include required air stream, lacking suitable barring down (periodically) and barring down after explosion have final weight 0.450 and 0.406 and 0.404 respectively and in Makran mine, there are 3 risks include suitable air conditioning of dirt, barring down after explosion and lacking suitable barring down (periodically) have final weight 0.534 and 0.508 and 0.470 respectively and all of these risks have known as final risks. On the other hand, responding to detected risks positively according to experts' opinions has been carried out for similar projects and the incident list of mine.
